2011-2012 Spring Sports Registration

For Grades 3-8

Ø                  2011-2012 SPRING REGISTRATION FEES

          Boys Baseball:    $100.00 per child

            Girls Fast-Pitch:  $100.00 per child

Ø                 HOW TO REGISTER

          1.  ONLINE @ www.stmichaelcyo.net    Click on “Online Registration”

              Complete online registration and submit a credit card payment.  Or, submit payment, via check and do the following:

                  **Make check payable to St. Michael Athletic Association

                  **Mail check to Karen Carter  (See address below.)

          2.  SUBMIT A CYO PHYSICAL FORM (2 pages) to Karen Carter

            A physical exam is required for each child participating in CYO sports.   The

                  physical is valid for one year from the date of the exam, and the entire sports season

                  must be within that year.  The CYO Physical Form must be signed and dated

              by your child’s physician.

              **The CYO Physical Form (2 pages), the form you should use, is available at

                     www.stmichaelcyo.net .    Click on “Forms”

                  **The second page of the Physical is a Medical Emergency Authorization form that

                     MUST be filled out and signed by the child’s parents/guardians.

                  **Your child will not be permitted to practice or participate in games

                until a current physical form is submitted.

(If you had submitted a CYO physical for a Fall and/or Winter Sport, your child will be

 covered as long as the physical is up-to-date. )

    3. SUBMIT FAMILY PARTICIPATION/UNIFORM DEPOSIT to Karen Carter

        Beginning with the Fall 2011 CYO Season and continuing with the Winter and Spring

          CYO Seasons, the St. Michael Athletic Association requires each CYO-participating

          family to submit one check for $75.00.             

·        It must be submitted prior to uniform distribution. 

·        It will be held until the child’s uniform is returned in full.

·        There are no volunteer hours to work in the Spring since there is no CYO

                         site to manage at St. Michael’s.

Registration Fee, CYO Physical Form, and Family Participation/Uniform Deposit Check must be submitted no later than MARCH 4th, 2012 for SPRING Sports.                                                                           

Ø      PLEASE NOTE:

1.  Athletes will not receive a uniform or equipment until all fees are paid and the CYO

      Physical Form (2 pages) is submitted.

2.   No child will be excluded from participating because of the parents’ inability to pay.  If

      you are in need of assistance, please inform the someone from the Board at the time of

      registration and alternate arrangements will be made.  All requests will be kept

      confidential.

3.   Late registrations will be accepted ONLY if there is a need to fill a team.

4.   Practices will begin the beginning of March.
